Case Management Program
Hearts for Homes offers ongoing case management for its clients to ensure that our seniors feel constant support from our organization.
Case Management Committee tasks include:
- Responsible for screening and interviewing homeowner applicants
 - Meet on a monthly basis to review and approved homeowner applications to forward to Project Management Committee
 - Maintain contact with homeowners before, during and after project completion
 - Invite homeowners to Heartbeat Testimonial Dinner
 - Responsible for Christmas Baskets Project
 - Assist with annual satisfaction survey
