Case Management Program

Hearts for Homes offers ongoing case management for its clients to ensure that our seniors feel constant support from our organization.


Case Management Committee tasks include:

  • Responsible for screening and interviewing homeowner applicants
  • Meet on a monthly basis to review and approved homeowner applications to forward to Project Management Committee
  • Maintain contact with homeowners before, during and after project completion
  • Invite homeowners to Heartbeat Testimonial Dinner
  • Responsible for Christmas Baskets Project
  • Assist with annual satisfaction survey
